What This Means For You

• The practice will cease seeing patients after 28 MARCH 2026.
• From that date our Doctors will no longer be available for consultations.

Continuing Care

Your medical records will continue to be securely stored in accordance with Australian Privacy Principles.

Transfer of Medical Records

To have your records transferred to the New Practices, you can complete a Transfer of Records at our Practice for Dr Suriyapalan or Dr Wimalaguna or complete the Transfer of Records form at the Medical Practices BELOW for your current Doctors so that we can action and forward on upon closure.

If you wish to continue with another GP entirely, simply complete a Transfer of Records Form at your nominated NEW Medical Practice and your file will be sent securely to your New Practice.
